Jockey Club Age-friendly City Project - Neighbourhood Care Scheme for Elderly Carers

Key components:
  • Conduct home visits to local residents and singleton and doubleton elderly living in old urban areas of Central and Western District, and assess by social workers to provide emotional support and counselling, and refer to suitable social welfare services if needed.
  • Provide activities and courses for elderly caregivers to relieve stress, including basic health checkups, mindfulness courses, and workshops on emotions and health.
  • Organise cross-sectoral professional exchange session and invite psychiatrists, nurses, occupational therapists and physiotherapists to provide information for elderly caregivers regarding management of physical and mental health of the elderly, daily home care, personal care, physical fitness, etc.
  • Invite members of home visit teams and residents of Central and Western District to set up a sustainable support network of “Neighbourhood Care” team.
  • Produce leaflets with practical guides on elderly care and organise “Neighbourhood Care” sharing session to promote the culture of mutual help in the community.
