Jockey Club Age-friendly City Project - Mobile Clinic for Healthy Lifestyle by Registered Nurse and Physiotherapist

Key components:
  • Produce a simple assessment checklist to help the elderly to understand the status of their body, mobility and social life.
  • Complete the checklist by the elderly with the assistance of volunteers and AFC Ambassadors, and follow up by nurses and physiotherapists to recommend plans for healthy living (including one-on-one consultations, treatment groups and seminars) based on the assessment results. Elastic bands and health kits (including information about the Jockey Club Age-friendly City Project, healthy lifestyle tips and practices for the elderly, support of family members, and community resources) will be distributed to the elderly so as to encourage them to live a health-lifestyle.
